When a crisis confronts an elder loved
one, family and friends want support they can trust, personal
understanding, and practical solutions to their special
situation.
Pathways provides personalized, professional services to
seniors and their families to identify resources, discover
care options, and make informed choices.
We know that every person is an individual, with specific
strengths and challenges. So we begin by listening carefully
and honoring each unique situation. Every person has his
or her own special plan and timing to uncover solutions,
restore hope and get back on track.
Pathways Social Workers provide counseling and consultation,
information and referral to community resources, and, when
funding allows, ongoing care management.
Special care is taken to address finances, medical issues,
support systems, housing, and spiritual concerns for seniors
and their families. In addition, caregiver issues are strongly
considered and options are developed.
Personalized, confidential service can be provided by telephone,
at the offices of the Foundation for Senior Living, or in
some cases in your home, Monday through Friday from 8 am
to 5 pm.
